New Patient Registration

Patients should obtain registration forms from reception or download these from the website,  see links below and complete and sign appropriately. This should be returned to reception as soon as possible so that we can obtain your medical records from your previous practice. We require you to have 2 forms of identification – a passport or driving license and a utility account with proof of your address. We will photocopy one of these forms of identification.

We operate a system in the Practice whereby you can see any of the General Practitioners and not solely the doctor whom you are registered with.

If you prefer to see the same Doctor then you can do so by arranging your appointments with your preferred GP.

Newly Registered Patients

All newly registered patients will be asked to complete a questionnaire and have a consultation with the Practice nurse for a small new patient medical to be completed. This involves a blood pressure, height, weight and urine check.

Medical treatment is available from the date of registration.
